Eligibility Criteria
To be eligible for Angel Baskets Holiday Program, you must reside in San Miguel County or the West End of Montrose County.
You may be required to verify enrollment in one of the following Social Services:
✔ Food Stamps (families & Seniors only)
✔ SSI
✔ Medicaid (children only MAGI)
✔ Medicaid Seniors
If you are eligible, please visit San Miguel County Social Services Office to fill out an application. Applications are also available through Tri-County Health Networks, the Wilkinson Public Library, the Telluride Medical Center and the Uncompaghre Medical Center.
Applications are available October 1.
PHARM AID
To be eligible for assistance with prescription and other pharmacy needs through the local participating pharmacies, you must reside in San Miguel County or the West End of Montrose County.
At the individual pharmacy’s discretion, you may be required to verify enrollment in one of the following Social Services:

✔ SSI
✔ Medicaid (children only MAGI)
✔ Medicaid Seniors
These are not strictly eligibility requirements. Financial need is determined on a case-by-case basis by our partner pharmacies based on their personal experience with and understanding of their customers.

NORWOOD
✔ County Residence in San Miguel or West End of Montrose. Proof of address may be requested.
SCHOOL SUPPLY PROGRAM
✔ Angel Baskets provides direct to school funding. Contact your school administrator.
